Summary:
Accountable to the high performing Health System's regional goals and objectives. Must have knowledge of areas including quality, risk, transitions of care, finance, regulatory compliance, and population health management. Directs the implementation, and ensures compliance with standards of practice. Responsible for evaluating the quality and appropriateness of care, the effectiveness and efficiency of systems within the home care setting, and instituting practice changes based on evaluation, and supporting interdisciplinary performance efforts within the continuum of care.
The Regional Director Home Care - Nursing focuses on the medical surgical, IV and maternal child patient throughout the system; develops strong relationships throughout CH in order to improve the quality of care and patient outcomes, while reducing readmissions. Regional Manager aligns services with the System's regions and physician service lines in all areas of responsibility. Promotes professional nursing and rehabilitation values, goals, and objectives among clinical staff and interdisciplinary team members. May include On-Call responsibilities evenings, weekends, and holidays.
Responsibilities:
EDUCATION- Graduate of an accredited School of Nursing required
- BSN required
- Maintains a current New York State Professional RN License required
- Masters degree in related clinical or management field preferred
- Advanced certification preferred
- Completion of a course in Public Health and or Principles of Management preferred
EXPERIENCE- Minimum four (4) years leadership/ management experience as evidenced by effective communication skills, conflict mediation skills, ability to develop and articulate vision for program of care, effective resource procurement and allocation, ability to mentor others, and ability to coordinate activities of others required
- 4 years current experience in clinical supervisor role or management position required
- Clinical background required
- Home Health Care experience preferred
- Quality/ process improvement experience preferred
- Marketing experience preferred
KNOWLEDGE, SKILLS AND ABILITY - Strong Clinical Quality Management skills
- Knowledge of cardiovascular disease and prevention of cardiovascular syndromes
- Knowledge of cardiovascular medications, their administration, and their side affects
- Demonstrated ability to develop strong relationships across CH at all levels

- Knowledge of rules and regulations governing Certified Home Health Agencies and New York State Nurse Practice Act
