Summary:
Works with the System Director of Pharmacy Services and site pharmacy leadership in the establishment and maintenance of a comprehensive Sterile and Non-Sterile Preparations Compounding Program that is compliant with all current regulations and standards set forth by regulatory agencies such as USP, Joint Commission, Department of Health, and State Board of Pharmacy. The Pharmacy Compounding Program Coordinator will oversee adherence with all of the requirements of the Compounding Program including coordination of training, staff competencies, staff education, as well as all required testing and certifications of the production facilities. The Pharmacy Compounding Program Coordinator will also be responsible for policy development, review, and update; all compounding related quality and patient safety initiatives including regular monitoring, quality metrics, adherence to department compounding quality standards and procedures, implementation of medication safety measures, monitoring of IV Room activities and report generation. The Pharmacy Compounding Program Coordinator will also participate in system wide committees related to the system compounding programs including cross system site inspections and review of certification reports and recommendation of remedial actions when necessary. The Pharmacy Compounding Program Coordinator also assists System Director of Pharmacy and site pharmacy leadership in maintenance of readiness for inspections by regulatory agencies and in ongoing compliance with all statutory regulations. In addition to duties associate with the Pharmacy Compounding Program, the Coordinator will maintain competence as set forth in the Clinical Pharmacist title for provision of operational and clinical pharmacy services when not engaged in coordinator duties.
Responsibilities:
EDUCATION AND EXPERIENCE- Doctor of Pharmacy degree (PharmD) with two years of experience in a hospital or infusion center with direct experience in sterile and non-sterile compounding, clinical pharmacy and order processing. Compounding must comprise at least 50% of the experience or
- Bachelor's Degree in Pharmacy with 4 years of experience in a hospital or infusion center with direct experience in sterile and non-sterile compounding, clinical pharmacy and order processing. Compounding must comprise at least 50% of the experience
- Thorough understanding of all applicable requirements from various regulatory bodies such as Joint Commissions, USP, DOH and NY State Board of Pharmacy
- Board Certification in Compounded Sterile Preparations Pharmacy (BCSCP) through the Board of Pharmacy Specialties (BPS) preferred at time of hire, but required within one year of hire
- Within 12 months of hire, maintains compliance with preceptor qualifications set forth by the American Society of Health-System Pharmacists (ASHP) if participating as residency preceptor
- Valid license and current registration to practice Pharmacy in New York State
- All Pharmacy Residencies require ASHP accreditation or candidate status
- Maintenance of Board Certification required
KNOWLEDGE, SKILL AND ABILITY- Ability to analyze/interpret and utilize results of various laboratory and microbiology tests and apply them to patient care in a clinical practice setting
- Excellent interpersonal skills and the ability to convey educational material in an understandable manner to patients and their families
- Ability to coordinate activities of multiple personnel and supervise their daily activities
- Ability to use hospital based information systems including Pharmacy, Laboratory, Patient Accounting and office systems
- Ability to operate ancillary patient care information systems in conjunction with operation of patient testing equipment
- Ability to communicate effectively with Physicians, Physicians Assistants, Nurse Practitioners, Nurses and other allied health professionals
WORKING CONDITIONS: - Travel to various locations for assignments, meetings, and educational programs required
