Summary:
This manager level position leads and supports the day-to-day operations of three successful care delivery programs, including the Health Home, Nurse Family Partnership and Starting off Right maternal care program. Responsible for daily program operations and clinical oversight, ensuring program compliance, adherence to local, state and federal regulations, along with implementation of strategies to support planned growth and program expansion. This position facilitates all aspects of program administration with responsibility for achievement of key clinical and financial metrics, program reporting, project management and budget development and implementation. Responsibilities:
EDUCATION- Bachelor's degree in a business, health related or human services field from an accredited program required
- Master's degree in a business, health related or human services field from an accredited program preferred
EXPERIENCE- Minimum five (5) years experience managing projects in complex business environments
- Demonstrated three (3) to five (5) years' experience with enterprise projects including over seeing a project portfolio (program) of many large projects simultaneously
- Minimum five (5) years experience with resource management of ten (10) or more staff (including vendors, outsourcing, interdepartmental, etc.) required
- Demonstrated financial experience managing budgets
KNOWLEDGE, SKILL AND ABILITY- Strong verbal and written communication skills
- Proven track record of delivery, project management, project team management, change management, and attention to detail
- Knowledge of Community Services to continue and expand a comprehensive care Health Home model that improves patient care delivery and creates partnerships between patients, physicians and medical care centers
- Knowledge and understanding of regulatory requirements, safety issues, and Joint Commission & Department of Health survey
- Influences and organizes the vision of the Health Homes to provide for future growth and developmen
- Sees inter-relationships and interdependencies and plans opportunities for synergy and integration; seeks root causes of issues/problems
- Facilitates strategic planning to identify and meet the short and long term business needs of Health Homes
- Provide leadership in the encouragement of creativity and innovation balanced with risk management in the continuous effort to create new standards of clinical quality in the provision of care to the Health Home clients
- Excellent computer skills
