Summary:
The Coordinator Admissions Community Based Care (CBC) has responsibility for the admissions process, ensuring compliance with applicable standards. Oversees the in- and outpatient functions, bed assignments, and completion of preliminary paperwork for entering patients. Works in collaboration with medical, nursing and accounting associates to ensure appropriate patient placement. Confirms that all insurance benefits coverage meets standards of admission as dictated by policy. Responsible for the development and implementation of customer service initiatives.
Responsibilities:
EDUCATION- Associates Degree required in Business or health-related field;
- Bachelors Degree preferred
EXPERIENCE- Minimum of one (1) year relevant work experience in health care related field required; two (2) years preferred
KNOWLEDGE, SKILL AND ABILITY- Possess the ability to ask pertinent questions to glean necessary information;
- Ability to make independent decisions based on knowledge, judgment and standardized guidelines;
- Ability to identify opportunities for improvement in operations, relationship services and amenities;
- Ability to implement programs and processes to address identified opportunities;
- Ability to lead change and to drive internal and external customer satisfaction;
- Possess working knowledge of how the interface between appropriate levels of care, payer regulations, and discharge planning influence the maintenance of budgeted case mix on the Sub-Acute Unit
- Knowledge of Geriatric rehabilitation
- Knowledge of payment structures in SNF setting, including knowledge of PPS and HMOs
- Strong customer service skills;
- Detail oriented;
- Excellent communication skills;
- Knowledge of Interqual criteria;
- Computer proficiency, including Microsoft Office products;
- Knowledge of Midas software preferred
WORKING CONDITIONS- May require travel between multiple locations;
- Willingness to work beyond normal working hours when necessary;
- May require variable schedule which includes weekends, evening, and holidays
- May be subject to hostile and emotionally upset patients, family members, and frequent interruptions throughout the workday
