Per Diem. Typically once or twice a month. It could be more it could be less. Times Vary between 7:00am - 6:00pm
Summary:
The Registered Nurse PRI Screener is responsible for completing a thorough and appropriate assessment on patients to determine the appropriate level of care needed. A PRI is a Patient Review Instrument which is a required medical screening tool that identifies whether a person is eligible or not for a skilled nursing home placement. The tool also identifies the level of care needed for the individual and is used to determine reimbursement to the facility. The incumbent also provides clinical leadership in a manner that reflects the values of the system.
Responsibilities:
EDUCATION
- Graduate from an accredited school of nursing, college or university required
- Current New York State RN license required
- BSN preferred
- Patient Review Instrument tool/screener certification by the DOH required
EXPERIENCE
- Minimum of two (2) years professional nursing experience required
- Previous experience in psycho social and skilled care assessments required
- Two (2) years experience in Home Health Care strongly preferred
KNOWLEDGE, SKILL AND ABILITY
- Proficient clinical and assessment skills
- Must be able to relate clearly and effectively in a professional manner
- Ability to display tact and sensitivity while maintaining confidentiality
- Excellent written and verbal communication skills
- Strong Organizational skills
- Ability to exercise independent judgment in executing responsibilities
