Summary:
The Referral Support and Transfer Center RN Supervisor is responsible to facilitate communication and coordination of patient referrals, transfers and admissions throughout the Catholic Health Integrated Delivery System. This position utilizes strong clinical knowledge and critical thinking skills to determine patient needs. Based on patient needs assessment, system specialization, and distribution protocols patient placement is accomplished safely in a manner meeting and/or exceeding expectations.
This position assigns patient bed placement for all Catholic Health hospital ministries and KMH Inpatient Rehabilitation Unit. Responsibility for timely, effective, and safe response to calls from patients in the Life and Home Health systems including documentation and escalation are integral to this position. Guidance and direction to the Transfer Center Specialist that fosters the position to perform at its highest level is expected. Associates work in collaboration with multiple departments within each ministry to foster relationships with referring facilities, physicians, and hospital staff. This position works closely with leadership teams at each facility to identify, trend, and report barriers to patient throughput.
Responsibilities:
EDUCATION- Graduate of an accredited professional nursing program
- Current NYS RN licensure and registration
- Bachelor's Degree in Nursing or organization management preferred
EXPERIENCE- Five (5) years RN experience, two (2) of which must be in critical care/ER
- Charge nurse, supervisory, or managerial experience preferred
KNOWLEDGE, SKILL AND ABILITY- Excellent communication skills and ability to:
- Utilize technology to enhance communication and to collect and report data.
- Obtain necessary information to enable prioritization of patient needs effectively and efficiently
- Provide clear direction
- Provide positive communication keeping patients and significant others participative in their plan for care and physician and ancillary staff with current assessments of responses and needs

- Evaluate patient needs and appropriateness for care setting
- Utilize professional judgment and autonomy in decision making, seeking assistance from management as needed
- Maintain current knowledge of professional and best practice standards, working together with team to assimilate same into professional care environment
