Summary:
The Comprehensive Stroke Center (CSC) Coordinator is responsible for the planning, implementation, and coordination of stroke services for The Comprehensive Stroke Center, in addition to the NYS designated and Joint Commission Primary hospitals system wide stroke program via a HUB and Spoke Model. The Joint Commission Accredited Comprehensive Stroke Center drives the delivery of care through evidence based practice, the most recent clinical practice guidelines, and meeting system goals in the development of standardized policies, system order sets, and care algorithms. The CSC Coordinator manages critical pathway timeframes and creates and implements delivery models for the transitions of complex patients between the system's hospitals and healthcare facilities. The Coordinator assumes day -to-day responsibility for process and performance activities as they relate to stroke care. The (CSC) Coordinator plans, implements, and evaluates system stroke clinical programs to achieve high quality, patient-focused outcomes which advance patient and family care in the treatment of stroke. Develops and delivers the most current evidence-based education targeting nursing and EMS providers in the local and regional communities, in addition to developing stroke care guidelines education for system hospitals and affiliates. This individual works collaboratively with Stroke Medical Director and Neuroscience System Director.

Responsibilities:

EDUCATION
  • Graduate of an accredited professional nursing program
  • BSN required
  • MSN required within three (3) years of hire
  • Preferred certification as Certified Neuroscience Registered Nurse (CNRN ®) OR five (5) years documented clinical experience working with stroke patients
  • Current RN licensure in New York State

EXPERIENCE
  • Minimum of three (3) years recent clinical nursing experience
  • BLS and ACLS validation must be obtained within twelve (12) months of assuming position

KNOWLEDGE, SKILL AND ABILITY
  • Strong understanding of neurological pathophysiology, assessment and treatment
  • Experience of expertise in developing patient and staff education materials, as well as providing education to individuals and groups
  • Evidence of ability to function with a high degree of independence while actively collaborating with health care providers system wide proving care to stroke patients
  • Exhibit excellent interpersonal skills: communication, problem solving, and conflict resolution
  • Interpersonal skills necessary to work productively with all levels of unit personnel, as well as physicians, families, visitors and to manage the activities of a diverse group of professional, nonprofessional and clerical employees
  • Planning skills to develop goals and objectives in concert with those of the hospital, and system, and implements action plans to meet those goals
  • Analytical ability to identify opportunities for improvements utilizing data analysis
  • Teaching abilities to promote optimum orientation and continuing education of staff
  • Has current knowledge of health care trends and issues and relates same to hospital and work area environment
  • Master communicator and strong negotiating skills
  • Ability to present at regional, national conferences

WORKING CONDITIONS
  • Subject to inside environmental conditions. Must be able to work in hectic environment making sound decisions at all times
  • Work is demanding and stressful at times. Visual and auditory acuity are essential
  • Considerable walking and standing during the tour of duty
  • Knowledge and utilization of proper body mechanics is essential as lifting, pulling and pushing patients and equipment are required
  • Works indoors in a well lighted, well ventilated area
  • May be exposed to communicable diseases
