Hours:
6:30a-2:30p, 7a-3p, 8a-4p/7.5 hours per day 5 days a week.

Summary:

Provides leadership with the System Director Cardiac Service Line for the Atrial Fibrillation Program of Excellence. He/she provides services for multidisciplinary, quality assured, and cost-effective care to patients who have experienced atrial fibrillation or other arrhythmias. Collaboration begins with referral physicians, cardiologists and other specialists to navigate the patient and family through the daily coordination of care across physician specialties and the health care team to develop evidence based arrhythmia management and care. The Atrial Fibrillation Program Coordinator will serve as a resource and liaison for Atrial Fibrillation by serving in the role of educator, quality improvement champion and consultant and coordinate appropriate patient referrals to the Atrial Fibrillation Program of Excellence Center. He/she also participates in the development of a marketing strategy, screening programs and other opportunities for program development.

Responsibilities:

EDUCATION
  • Graduate degree as a Nurse Practitioner with valid New York State license
  • Advanced Cardiac Life Support Certification within one (1) year of employment
  • Basic Life Support and other advanced training as appropriate

EXPERIENCE
  • At least one (1) year experience in Cardiology and/or Electrophysiology required
  • Previous leadership experience preferred
  • Previous experience with projects or program start-up preferred
  • Ability to read, write, and explain or describe moderately complex information to other hospital staff, with attention to detail

KNOWLEDGE, SKILL AND ABILITY
  • Able to constantly concentrate on fine detail with constant interruption
  • Able to frequently attend to task/function for more than 60 minutes at a time
  • Skill in organizing resources and establishing priorities
  • Ability to supervise and rain employees, to include organizing, prioritizing, and scheduling work assignments
  • Knowledge of project management principles, practices, techniques, and tools
  • Knowledge of computer billing systems and IS applications
  • Ability to foster a cooperative work environment

  • Skill in preparing and maintaining patient records
