Summary:
Collaborates with members of the health care team to provide Medical Nutrition Therapy (MNT) through the Nutrition Care Process including nutrition education and counseling to patients. Identifies patients at nutrition risk and assesses nutrient requirements according to age and disease specific needs. Develops a Nutrition Diagnosis and implements nutrition care treatment plans for all assigned patients. Continuously evaluates and monitors the effectiveness and outcomes of MNT intervention.
Responsibilities:
EDUCATION- BS Degree in Nutrition/Dietetics plus internship or other education to meet requirements for registration with the Commission on Dietetic Registration (CDR)
- Registration with the CDR
- Registered Dietitian or registration eligible. Registration must be obtained within 6 months of employment
- Certified Dietitian-Nutritionist (CDN) with New York State to obtained within 1st year of employment
- Active membership with the Academy of Nutrition and Dietetics preferred
EXPERIENCE- 1 year experience preferred
KNOWLEDGE, SKILL AND ABILITY- Must possess or have ability to learn basic computer skills
WORKING CONDITIONS - Occasional exposure to one or more mildly unpleasant physical conditions
- Travel between Catholic Health System facilities may be required
- Occasional evenings may be required for nutrition counseling appointments or classes
