Summary:
JOB SUMMARY:
Responsibilities include the end user support of Catholic Health's Electronic Medical Record (EMR) for the clinical system. Interacts directly with clinical associates and managers by making regular rounds in order to answer questions, improve correct use of, and comprehension of the EMR clinical system. Communicates changes, updates, and additional implementations of the EMRclinical system. Supports the correct, efficient, and standardized use of clinical systems appropriate to their job duties. Supports management in chart audits, associate adherence to clinical standards, education of clinical skills and EMR applications. Carries a nursing caseload that may include home visits for assessments, nursing home case communications. Responsible for assessment of health needs and contributing to the plan of care, initial implementation of nursing care plans, and providing nursing care, as well as periodic re-evaluation of individual and family nursing needs. Supervises home health aides in the home, clinic, and day center to ensure regulatory compliance and quality while maintaining an optimal level of independence for participants.
On-call responsibilities are included.
Responsibilities:
EDUCATION
- Registered Nurse must be currently licensed in New York State
- Graduate from an accredited school of nursing currently registered in New York State
- BSN or BA in Healthcare strongly preferred
- BLS validation and revalidation in accordance to AHA standards
EXPERIENCE
- Minimum of three (3) years Healthcare experience with use of Electronic Medical Record required
- Minimum of one (1) year teaching/training required
- Minimum of two (2) years Home Care, long term, or medical surgical care experience preferred
- Experience with patient assessment instrument- NYS Uniform Assessment System (UAS).
- Preferred at least one year experience working with the frail elderly.
KNOWLEDGE, SKILL AND ABILITY
- Outstanding and proven skills to interact directly with clinical customers in a face-to-face live environment
- Knowledge of clinical processes and operations
- Technical knowledge of EMR computer applications preferred
- Good verbal and written communications skills
- Demonstrated skills with personal computer applications, including Windows, and Microsoft Office applications
- Ability to present educational materials and nursing skills competencies
- Good organizational and analytical skills
- Ability to facilitate discussions, brainstorming sessions and team processes
- Ability to establish good working relationships with staff at all levels, including physicians, nursing and ancillary staff, and clinical management
- Ability to work independently and productively in a very fluid work environment with minimal direct supervision
- Ability and means to travel among CHS sites, including a valid NYS driver's license
