Summary:

The Vice President Population Health & Clinical Transformation for Catholic Medical Partners (CMP) is responsible for developing and implementing population health management and operational strategies of Catholic Medical Partners (CMP) to ensure organization goals are met or exceeded. As a member of the senior leadership team, the Vice President Population Health & Clinical Transformation will align clinical integration programs to effectively address the current and future needs of the organization in accordance with the organization's mission, vision and strategic plan. Quality improvement, utilization and care management initiatives will align with evidence based guidelines, medical policy and NCQA patient centered medical home standards in support of our over 900 physician members and Catholic Health's integrated network. Responsible for strengthening the coordination of care within the CMP network to meet or exceed contractual requirements among payers to improve patient experience of care and quality of care while reducing the total cost of care. Will lead performance improvement and operational efficiencies by leveraging health information technology and analytics in clinical quality, network integrity, clinical transformation and care management. Management responsibilities include all aspects of physician relations, membership, network optimization, communications, and daily operations. The Vice President Population Health & Clinical Transformation works in close collaboration with the Chief Medical Officer as a functional management dyad. The Vice President Population Health & Clinical Transformationis also responsible to carry out the project management requirements supporting the ten clinical integration initiatives for Delivery System Reform Incentive Program (DSRIP).

Responsibilities:

EDUCATION
  • Bachelor of Science in Nursing
  • Masters degree in public health, health care administration or ten (10) years commensurate professional responsibility

EXPERIENCE
  • A minimum of ten (10) years of relevant health care leadership experience in the areas of quality, utilization, and care management with a focus on program design, measurement, and continuous improvement
  • A minimum ten (10) years with staff recruitment, development, retention and succession planning
  • A minimum of eight (8) years in clinical program and project management
  • Experience leading cross-functional/disciplinary teams including physicians and staff through significant change
  • Experience in managed care, utilization management, case management, chronic condition management, health promotion and/or quality management programs

  • Demonstrates strong time management and organization skills, can orchestrate multiple activities at once. Uses resources effectively and efficiently
  • Excellent knowledge on how to design and implement clinical integration and care management programs to be successful in value based contracts
  • Knowledge of evidence based guidelines and objective measures of quality care
  • Understanding and use of information technology systems to support population health management in value based contracts
  • Ability to engage in continuous improvement activities (LEAN and six sigma methodologies)
  • Excellent written and verbal interpersonal communication skills
  • Ability to work with the Board of Directors and committees of the board
  • Project management skills including accurate resource, timeline and milestone objectives
  • Excellent problem solving skills and knowledge of workflow charting to optimize operational reliability
