Summary:

The Clinical Transformation Specialist I will be responsible for defining and driving Catholic Medical Partners (CMP) physician offices in their improvement of quality of patient outcomes, patient experience of care, and utilization through the use of health information technology, use of team based care, and overall practice process improvement; thereby preparing practices to continue the transformative activities that demonstrate consistent improvement on performance and quality measures. The role is responsible for traveling and meeting with the staff at CMP physician offices, reviewing office workflow, teaching physicians & office staff to correctly document data utilizing EHR, how to run and analyze quality reports utilizing their EHR and measurement of their improvement in utilizing systems, as well as utilization of prevention and chronic illness quality reports.

Responsibilities:

EDUCATION
  • Bachelor's degree required; preferably in a healthcare related field

EXPERIENCE
  • A minimum of two (2) years of relevant work experience in a healthcare, physician office based experience preferred
  • Previous EMR experience with Medent, eClinicalWorks, or Epic strongly preferred
  • Previous experience with quality measures and national recognition programs (i.e. PCMH, CPC+, HEDIS, NQF) preferred

KNOWLEDGE, SKILL AND ABILITY
  • Ability to learn, apply and teach new concepts, programs, and technology solutions
  • Outstanding and proven skills to interact directly with customers in a face-to-face live environment
  • Outstanding and proven skills to organize and prioritize tasks in a face-to-face live environment
  • Able to work in a highly complex integrated health system with multiple demands for support and strategic priorities
  • Excellent verbal and written communication skills required
  • Demonstrated interpersonal communication skills
  • Ability to communicate complex concepts in a clear, concise manner
  • Knowledge of population health management principles
  • Knowledge of quality care measures that drive performance
  • Previous project management experience strongly preferred
  • Ability to establish effective working relationships
  • Ability to meet deadlines consistently
  • Ability to maintain confidentiality of sensitive and confidential information

  • Excellent computer software program skills required (Microsoft Word, Excel, Outlook, and PowerPoint)
  • Driver's license and reliable car

WORKING CONDITIONS
  • Associate must have reliable transportation and be willing to travel on a regular basis up to two (2) hours from CMP office location to meet with physician practices on a regular basis
  • Normal heat, light space, and safe working environment; typical of most office jobs
