Job Description

Status: Full Time Facility: Sisters of Charity Hospital St Joseph Campus

Work Schedule: Days and Evenings Shift: 1:

Summary:

Collaborates with members of the health care team and provides Medical Nutrition Therapy (MNT) and Diabetes Self Management Education (DSME) to patients in an outpatient and/or inpatient setting. Conducts thorough, individualized health risks assessments and reassessments of patients being referred including assessing diabetes self management skills and nutrient requirements according to age and disease specific needs. Guides the patient in setting and prioritizing individualized behavioral goals based on assessment of needs and personal and cultural preferences. Develops and implements diabetes and/or nutrition care plans for patients/clients. Continuously evaluates and monitors the effectiveness and outcomes of MNT/DSME intervention and reports back to the referring physician.

Responsibilities:

EDUCATION
  • B.S. Degree in Nutrition/Dietetics from an accredited educational program plus successful completion of dietetic internship, or equivalent which would qualify for Professional Registration with the Commission on Dietetic Registration;
  • Master's Degree in Dietetics, Education or Counseling preferred
  • Registered Dietitian/Nutritionist (RDN) required; active and on-going registration with the Commission on Dietetic Registration
  • Certified Diabetes Educator (CDE) required; active and on-going certification with the American Association of Diabetes Educators
  • Certified Dietitian/Nutritionist (CDN) required; active and on-going certification through the NYS Office of Professions; and
  • Active membership with the Academy of Nutrition and Dietetics (AND) preferred
EXPERIENCE
  • 3 -5 years of clinical experience as an RDN required;
  • 1-3 years of inpatient/outpatient teaching experience required

KNOWLEDGE, SKILL AND ABILITY
  • Strong Microsoft Office skill set
  • Strong written and verbal communication skills

WORKING CONDITIONS
  • Occasional exposure to one or more mildly unpleasant physical conditions;
  • Travel between Catholic Health facilities may be required;
  • Required to transport educational materials; and
  • Occasional evenings required for diabetes and/or nutrition counseling appointments or classes
