Summary:


The social worker provides comprehensive social work services to women, babies, and their families in primary care. The maternal child social worker may also provide social work and discharge planning to mothers and their babies in the hospital.

As an active member of the interdisciplinary team the social worker affects a process of assessment, planning, facilitation and advocacy to meet bio-psychosocial needs of mothers and babies with attention to quality, thoughtful use of resources and optimum patient experience.

The social worker demonstrating the mission, values and mission of Catholic Health builds and maintains professional, resource based relationships across the Catholic Health continuum of care and within the community.

Responsibilities:


EDUCATION
  • Bachelor of Social Work (B.S.W.) from a college or university accredited by the Council on Social Work education.

EXPERIENCE
  • Minimum one year B.S.W. social work experience working with women and children; preferably in a health related setting

KNOWLEDGE, SKILL AND ABILITY
  • Comprehensive knowledge of the spectrum of psychosocial issues that impact the health of pregnant women and their babies
  • Extensive knowledge of community resources and services specific to women, children, and families
  • Strong ability to work within the social work role on an interdisciplinary team
  • Ability to manage a high volume of psychosocially complex cases while concurrently maintaining optimal patient experience
  • Excellent interpersonal, communication, networking, and advocacy skills

WORKING CONDITIONS
  • Varied hours and days at times, to meet the needs of patients/families and primary care operations
  • Occasional travel between Catholic Health facilities as needed
  • Normal heat, light, space, and safe working environment typical of most office jobs; and
  • Occasional exposure to one or more mildly unpleasant physical conditions
