The Nurse Practitioner, Clinical Coordinator of the Dialysis Access Center (DAC) is responsible for providing patient care coordination across the total DAC patient care continuum. The Clinical Coordinator will serve as the primary source of collaboration between the DAC care team and outside dialysis units in the community. The Clinical Coordinator will work with DAC Medical Director to implement best practices, protocols, patient assessment, triage of dialysis unit phone calls, daily coordination of the patient schedule and maintain open communication with clinic & office staff. Responsibilities:
EDUCATION- Graduated from an accredited college or university with a Master's Degree as a Nurse Practitioner
- Certification in nephrology or hemodialysis preferred
- AACN or AANP Certified Nurse Practitioner
- Valid NYS Licensed RN
- Advanced Life Support Certification required
- Basic Life Support Certification required
EXPERIENCE- Experienced hemodialysis professional required
- Demonstrated proficiency in vascular access care and management preferred
- Minimum five (5) years of clinical and programmatic work experience as a Nurse Practitioner with progressive work experience in the care of renal access patients and performing vascular access and endovascular procedures
KNOWLEDGE, SKILL AND ABILITY- Excellent verbal and written interpersonal communication and customer service skills
- Solid computer skills
WORKING CONDITIONSENVIRONMENT- Subject to clinical environmental conditions
- Knowledge and utilization of proper body mechanics is essential as lifting, pulling and pushing patients and equipment are required
