Summary:


The Clinical Nurse Educator position is responsible for providing Catholic Medical Partners-Clinical Transformation team and CMP provider practices and their staff development expertise as an educator, consultant, facilitator, change agent and researcher. This occurs through the facilitation of the educational process assessment, development, planning, implementation, and evaluation of competencies, continuing education to meet the clinical learning needs of Catholic Medical Partners-Clinical Transformation team and CMP practice members. Works as a member of the Clinical Transformation team with a keen interest in quality metrics and payer reimbursement models. Additionally, responsible for the onboarding of the Care Management roles in the primary care offices that are members of CMP. In collaboration with CMP leadership and others, coordinate and establish the education plan on an annual and ongoing basis. Instrumental in creating a professional climate at CMP.

Responsibilities:


EDUCATION
  • Current New York State RN License
  • MSN, MS in Education or related field preferred
  • BSN and must obtain a Master's degree within three (3) years of hire

EXPERIENCE
  • Minimum of five (5) years clinical experience required
  • Minimum of three (3) years of relevant staff education work experience within community primary care or specialty practices
  • Background/experience in Care Management

KNOWLEDGE, SKILL AND ABILITY
  • Strong experience in learning assessment, curriculum development, care plan development
  • Excellent written and verbal interpersonal communication skills
  • Solid PC skills utilizing Microsoft Word, PowerPoint and Excel software applications
  • Ability to travel between the various practice sites
  • Carry equipment/supplies from one facility to another
  • Knowledge of population health management principles and social determinants of health
  • Knowledge of quality care measures that drive performance
