Summary:

The Clinical Transformation Specialist II is a specially trained individual who is responsible for developing a comprehensive population health management approach for CMP practices in alignment with the CMP Clinical Integration (CI) Program to meet the Quadruple Aim (improved quality of care, improved affordability, improved patient satisfaction and improved provider/staff satisfaction).This position is a leadership role and coaches practice physicians and staff to improve patient outcomes by developing skills in process improvement, team-based care, encouraging patient engagement, and analyzing quality data and measurements. The Clinical Transformation Specialist II is committed to leveraging health information technology for quality improvement, research, and practice transformation. This position provides leadership and expertise in the development and implementation practice specific work plans for clinical process improvement, optimizing team based care, adherence to evidence based care, and patient engagement. The Clinical Transformation Specialist II will work as part of an interdisciplinary team to create and deliver products and services including data analysis and interpretation, project plans, tool kits, and evaluation materials to drive and inform future practice performance improvement initiatives. The Clinical Transformation Specialist II provides mentoring and training for Clinical Transformation Specialist I associates. This position performs duties with minimal supervision.

Responsibilities:

EDUCATION
  • Bachelor's degree required; preferably in a healthcare related field

EXPERIENCE
  • Three (3) - five (5) years practice transformation experience required
  • A minimum of four (4) -eight (8) years of relevant work experience in a healthcare related background
  • Previous EMR experience with Medent, eClinicalWorks or Epic required
  • Previous experience with implementing quality improvement and national recognition programs (i.e. PCMH, CPC+, HEDIS, NQF) required

KNOWLEDGE, SKILL AND ABILITY
  • Understanding and utilization of current information technology systems designed to support practice transformation, ability to learn new systems
  • Knowledge of how to implement process and workflow changes consistent with NCQA & CPC+ standards
  • Knowledge and understanding of value based reimbursement models (i.e., partial capitation, financial risk arrangements)
  • Demonstrated ability to learn, apply and teach new concepts, programs and technology solutions
  • Demonstrated understanding of how quality care measures drive top performance in publicly reported data sets
  • Demonstrated ability to educate office based care teams to utilize data in developing their practice improvement plans
  • Able to work in a highly complex integrated health system with multiple demands for support and strategic priorities
  • Understanding of population health management principles
  • Knowledge of prevalent chronic conditions
  • Previous multi-location project management experience required
  • Ability to communicate complex concepts in a clear, concise manner
  • Demonstrated interpersonal communication and team-building skills
  • Outstanding and proven skills to interact directly with customers in a face-to-face live environment
  • Outstanding and proven skills to organize and prioritize tasks in a face-to-face live environment
  • Demonstrated ability to be creative, innovative, collaborative, self-directed, ability to take initiative, motivate and inspire others
  • Strong ability to teach staff, including, mid-levels and physicians to deliver high quality coordinated care in the clinical office
  • Ability to establish effective working relationship with CMP physicians and to promote clinical transformation
  • Ability to develop, deliver and facilitate group presentations
  • Ability to meet deadlines consistently
  • Ability to maintain confidentiality of sensitive and confidential information
  • Excellent computer software program skills required (Microsoft Word, Excel, Outlook and PowerPoint)
  • Driver's license and reliable car

WORKING CONDITIONS
  • Associate must have reliable transportation and be willing to travel on a regular basis up to two (2) hours from CMP office location to meet with physician practices on a regular basis
  • Normal heat, light space, and safe working environment; typical of most office jobs
