Summary:
In concert with the Director of Care Management, site leadership, and the site Manager of Care Management, this position is responsible to manage and lead the overall functions of the social work team to ensure consistent, effective and efficient site specific transitioning of patients across the care continuum in an integrated delivery system. Accountable for success in achieving site specific and system wide deployment of best practice care models for social work and discharge planning and discharge disposition and care coordination across our continuum. Accountable for achievement/success in meeting/exceeding system wide social work performance metrics, productivity standards, patient experience, as well as select quality /core measures. The Supervisor, Social Work may serve as a field educator/supervisor for undergraduate and graduate social work interns from a school accredited by the Council on Social Work Education.
Responsibilities:
EDUCATION- Bachelor's degree in Social Work required from an accredited school of Social Work and at least one (1) year of experience of supervising clinical social work teams
- Master of Social Work (MSW) from a school accredited by the Council on Social Work Education
- Current NYS Master of Social Work license (LMSW) or current NYS Clinical Social Work license (LCSW) desired
- Care Management Certification preferred
- NYS Screen Certification preferred
EXPERIENCE- At least one (1) year of experience of supervising clinical social work teams aimed at improving the delivery of social work services using evidence based principles and change management
- Three (3) years of experience in a social work position with experience with discharge planning, transitions of care and discharge disposition preferably in an acute care environment
- Proficiency in social work standards and regulatory requirements required

- Extensive knowledge and innovative strategies for medical social work, discharge planning principles /practice
- Extensive knowledge and fluency of NASW, NYS DOH, CMS, JCAHO, third party regulations and compliance standards as it relates to social work/discharge planning/transitions of care
- Strong interpersonal/intrapersonal relationships with the care management team, ancillary departments and medical leadership teams
- Experienced subject matter expert and thought leader on social work, care coordination, discharge planning, strategies and initiatives which includes keeping abreast of traditional and innovative developments in the delivery and management in an integrated health care delivery system
WORKING CONDITIONS- Routine travel between CH facilities
